Developed by Meadowlark Hills and Action Pact, Inc, and funded by the Commonwealth Fund, Sunflower Foundation and the Kansas Department on Aging, Household Matters is an integrated toolkit of materials and systems for use by long-term care providers (and others) with a stake in transforming the culture of nursing homes in a sustainable fashion.

Taken together, the toolkit's components provide an explicit roadmap for how an organization can shape itself to operate in a resident-directed service model with a well-trained, empowered, and satisfied workforce. The packaged toolkit will not only assist in assuring culture change sustainability, but will be a vehicle also for increasing the rate of culture change penetration in the long-term care community.

Packaged in a sleek, hardboard "briefcase" with a carry-along handle, the toolkit is perfectly suited for both a resource bookshelf, or a mobile resource to move between departments or clients.

The Household Matters toolkit contains the following:

  1. In Pursuit of the Sunbeam: A Practical Guide to Transform Institutions to Homes. A leadership book by Steve Shields & LaVrene Norton that assists providers by providing a change framework based on the Norton/Shields Change Matrix: Self, Leadership, Organizational and Environmental Transformation.

  2. Daybreak: Creating Home. A CD-ROM containing a comprehensive set of policies and procedures shaped for household life.

  3. Midday: Living & Working in Harmony. On CD-ROM, an integrated human resource system that reflects the values of the new model.

  4. Evensong: Reflecting on Quality. A CD-ROM with a system of team-based continuous quality improvement.

  5. Training Guide for Self-led Teams. A comprehensive 168-page workbook filled with exercises and discussions to flesh out the concepts that are the foundation of teamwork in the new model.

  6. 3 DVDs of video vignettes to use in training. Supporting and illustrating concepts ranging from Hiring and Orientation to Dining to Learning Circles.

Household Matters is a comprehensive template of methodologies and support systems - a template to initiate and sustain change to transform institution to home in the nursing home.

There are several potential audiences for this product, the principal one being organizations such as nursing homes and assisted living facilities which provide institutional, long-term care services. Additionally, this toolkit will greatly benefit both consultants in long-term care, and training programs for LTC administrators.

Ultimately, the future of long-term care lies in preparing administrators to lead those nursing homes which have become person-directed, or to lead nursing homes through the culture change process. The multi-media resources included will be invaluable to consultants and training programs enhance their services and effectiveness in helping organizations reshape themselves.
