This is a guide to help you really get to know a person. The Activity Interest Assessment is long, but don't let the length scare you! Aren't you tired of short assessments that typically only mimic the MDS and at the end we are still left wondering, "But, who is she?" Let's get beyond the institutional way of just filling out a form because we have to. This guide has been designed to help really get to know a new person coming to live in your midst (a new neighbor instead of a "new admit"). Isn't that what any of us would want - to be known? Really utilize this tool in short intervals to build rapport and relationships, that's what surveyors do with their many resident interview questions.
Interest prompts are included to help you find out more. Instead of just having a prompt entitled "Cards" for the assessor, we listed every card game we could think of. This helps prompt the person's memory regarding past activities enjoyed. It also gives the opportunity to ask the person if they might like to try to learn something new, i.e. a card game they have never played.
Included in this assessment are also common culture change practices around getting to really know a person including things like the person's typical routines, daily pleasures, and what brings meaning and purpose into their lives. Prompts are included to ask about loneliness, boredom and helplessness, the "three plagues" of institutionalization identified by the Eden Alternative.
In order to help us on our journey away from institutional care and towards person-directed care, we've developed a list of Principles of Excellence to consider adopting, following, hanging up in your office for all staff to see. These challenge us to rise higher, treat getting to know someone with utmost honor, turn the assessment process into a more normal "chit chat" than filling out a form; and you'll get to know someone "over coffee" rather than "over a form."
